Our Team

David Baldanza, P.T., DPT, CM

David is a physical therapist and the center manager at the Craig Road location. He earned his degree from Daemen College in Amherst, New York, and has been practicing since 2016. David holds a certification in dry needling and has specialized experience in cardiopulmonary and sports orthopedics. He is passionate about helping patients recover and improve their quality of life. In his free time, David enjoys working out and spending time with his family and dogs.

Jasmine Balen, P.T., DPT

Jasmine is a physical therapist who earned her degree from Utica University. She is certified in dry needling and trained in the Mulligan concept of mobilization with movement, bringing advanced manual therapy techniques to her center. Since beginning her career in 2021, Jasmine has developed a strong interest in orthopedics and hands-on rehabilitation. She is passionate about helping patients restore function and improve quality of life through personalized care. In her free time, Jasmine enjoys staying active by going to the gym and hiking scenic trails.

Zachary Dao, P.T., DPT

Zachary earned his degree from Franklin Pierce University and became certified in dry needling in 2025. He has a strong passion for sports rehabilitation and manual therapy. Outside of work, Zachary enjoys staying active at the gym, traveling and spending quality time with friends and family. A fun fact about him: his favorite music genre is reggaetón.

Denis Milian Shostak, P.T., DPT

Denis holds a degree from the University of Miami and is certified in dry needling and Parkinson's wellness recovery. He is passionate about working with older adults and individuals with neurological impairments, focusing on improving mobility, function and overall quality of life. Denis enjoys calisthenic training, running and playing or watching soccer and volleyball.

Richard Deagrella, PTA

Richard earned his physical therapist assistant degree from Pima Medical Institute and the College of Southern Nevada. He has specialized training in blood flow restriction therapy, concussion management, lumbar spine therapy, taping techniques, plyometrics and manual therapy. Richard is dedicated to providing evidence-based care tailored to each patient's needs. Outside of the clinic, he enjoys playing golf.

Tiffany Kesick, PTA

Tiffany holds a degree from Utah Tech University and brings more than two years of experience in the physical therapy field. Tiffany enjoys building strong connections with her patients and helping them reach their functional goals. She loves spending time outdoors, hiking, swimming and boating.

Antonio Gonzalez – Anaya, OTR/L, CHT

Antonio earned a bachelor's degree in psychology in 2015 from the University of Nevada, Las Vegas, and a master's degree in occupational therapy in 2018 from Touro University. Antonio became a certified hand therapist in December 2021. He is bilingual in English and Spanish. Antonio is passionate about working with patients to enhance their functional abilities while developing interpersonal relationships. He is devoted to helping patients achieve their highest potential through the use of evidence-based interventions and client-centered care. Antonio is eager to expand his knowledge about the upper extremity by attending multiple, continuing education courses. He enjoys hiking, going to concerts, drawing and the New Orleans Saints.

Guadalupe Ortega, OTA/L

Lupe holds a degree from the College of Southern Nevada and brings more than 25 years of experience specializing in the rehabilitation of the upper extremity. He is highly skilled in treating a wide range of conditions involving the shoulder, elbow, wrist and hand, with a strong focus on restoring function and improving quality of life. Lupe enjoys playing guitar, riding motorcycles, gardening, watching movies and documentaries and listening to music.

Community Involvement

Dignity Health Physical Therapy provides athletic training coverage for the Clark County School District, including Bishop Gorman and Faith Lutheran, servicing over 300,000 students.

Our athletic trainers work with the student athletes, their parents, coaches and team physicians to help injured athletes return to their sport in a safe, efficient manner. With 20 outpatient physical therapy centers throughout Las Vegas, our team of physical therapists assist in the recovery and prevention aspect alongside our athletic trainers.

OrthoVive Joint Health

Dedicated to managing discomfort and loss of function, OrthoVive Joint Health meets you where you are in your osteoarthritis, or degenerative joint disease journey, treating the condition and extending the life of your joint. Our therapists can enhance the results of non-surgical treatments and when surgery is needed, serve as your guide and coach maximizing the use of your new joint.
